Provider Collaboration Manager, IDD - Remote in the Nashville, TN area. 


The Provider Collaboration Manager will spearhead Amerigroup TN’s development of IDD Primary Care Centers of Excellence partnerships. This highly driven individual will support statewide collaborations towards the creation of Medical Health Homes for individuals with Intellectual and Developmental Disabilities.

Receives a moderate level of guidance and direction in providing operational and administrative support for the implementation, auditing and issue resolution for payment innovation programs.


Ideal Candidate Background/Skillset:


The ideal candidate will possess experience working with Individuals with Intellectual or Developmental Disabilities (IDD) whether through in-person care and support or working at an office/organization serving individuals with IDD (e.g. experience working as a Direct Support Professional, Support Coordinator, Case Manager, Social Worker or related profession).  This is a must for this role.   


This candidate must be independent (responsible to set own meetings and manage schedule); have a passion for improving the system and support for the people with IDD, and the leadership ability to offer peer to peer support by providing tools, knowledge and resources. 


In addition to required knowledge of IDD for the position, this candidate must also have the necessary business acumen and professional skills to collaborate with Providers with the duties listed below.  The successful candidate will have a well-balanced combination of IDD experience AND network development/analytics.

In-state travel in Tennessee required (approx. 60%). 

This position is remote but must be located in/near the Nashville, TN area.


Primary duties may include, but are not limited to: 

  • Provides support for assigned provider contracts.   
  • Obtains and analyzes financial performance and quality metrics.  
  • Support and facilitates value based JOCs (Joint Operating Committee meetings). 
  • Develops and maintains contact database and disseminates communications to providers as necessary. 
  • Aggregate and analyze provider data form various sources to recommend and implement solutions to assist with provider collaboration activities. 
  • Collaborates with provider contractors and network relations reps to meet provider service needs. 
  • Encourages providers to act on patient outreach and enhanced communication opportunities. 
  • Identifies areas of training that support current trends in services and supports for people with IDD.
  • Engages creatively with providers who work to support people with IDD that maintain a focus on independence, health and wellbeing.
  • Continues to build knowledge of the organization, processes and customers. 
  • Performs a range of time sensitive deliverables supporting contractual requirements.



  • Requires a BA/BS degree; 3 years’ experience in data base maintenance related to health care; or any combination of education and experience, which would provide an equivalent background. 

  • Provider environment experience .

  • Healthcare experience preferred. 

  • Value Based Care Programs background preferred

  • In-state travel in Tennessee required (approx. 60%).

  • Good communication, Self-Starter and able to work independently required.
