The Mgr CareMore CM Operations is responsible for providing leadership to assigned region for all Care Management functions.
Primary duties may include, but are not limited to:
•Manages and acts as an advisor, giving direction to all care management staff, resolving issues/problems as needed.
•Hires, trains, coaches, counsels, and evaluates the performance of direct reports.
•Develops and implements organizational strategies, policies and procedures.
•Improves the operational system, processes, and policies in support of CareMore’s mission.
•Manages and increases the effectiveness and efficiency of Care Management staff, through process improvements.
•Serves as liaison between Care Management and other internal departments, health plan partners, and community partners (hospitals, community based organizations, etc.).
•Hires, trains, coaches, counsels, and evaluates the performance of direct reports.
•Assists in creating annual Case Management (CM) work plan, quarterly reports, and annual evaluations.
•Participates in management team meetings and assists with leading team meetings.
Qualifications
- Requires a Licensed Master Social Worker or Registered Nurse with 5 years of experience in care management or any combination of education and experience which would provide an equivalent background.
- 3 years of management experience preferred.
- Current unrestricted RN or LMSW license in applicable state(s) required.
