The Nurse Disease Management l will be responsible for telephonic outreach, enrollment and management of Disease Management members. Provides education and coordination to address gaps in care for multiple managed conditions to ensure cost effective and efficient utilization of health services. Conducts assessment, planning, implementation, coordination, monitoring and evaluation of members through telephonic/call center setting. Responsible for participating in delivery of patient education and disease management interventions, performing health coaching for members, across multiple lines, and health improvement / management programs for chronic diseases.

Primary duties may include, but are not limited to:  

  • Conducts behavioral or clinical assessments to identify individual member knowledge, skills and behavioral needs.
  • Identifies and/or coordinates specific health coaching plan needs to address objectives and goals identified during assessments.
  • Uses motivational interviewing to facilitate health behavior change.
  • Monitors and evaluates effectiveness of interventions and/or health coaching plans and modifies as needed.
  • Implements and/or coordinates coaching and/or care plans by educating members regarding clinical needs and facilitating referrals to health professionals for behavioral health needs.
  • Directs members to facilities, community agencies and appropriate provider/network.
  • Interfaces with provider and other health professionals to coordinate health coaching plan for the member.

Location:  [email protected] in NV, OH,GA, IN, MO, VA, CAMust reside within a 2 hour commute of an Anthem Office.

 Work Hours: Scheduled 8 hour shift between 10:30am – 11pm Eastern Time.

Potential shift options:

10:30am-7:00 pm ET

11:00am-7:30pm ET

12:30pm-9:00pm ET

2:30pm-11:00pm ET

Available shifts to be discussed at time of interview.

Minimum qualifications

  • Requires an AS/BS in nursing; 2 years of condition specific clinical or home health/discharge planning experience; or any combination of education and experience, which would provide an equivalent background.
  • Current unrestricted RN license in state of residence required.
  • Prior disease management experience preferred.

Preferred qualifications

  • Ability to obtain multi-state license.
  • Experience with Coronary Artery Disease and COPD a plus.  
  • Ability to meet necessary internet speed requirements and adhere to work at home agreement.

Annual Salary Range*: $58,720 - $77,070

Actual compensation may vary from posting based on geographic location, work experience, education and/or skill level.
