Field Based visiting 8 – 10 patients daily within assisting living, SNF, memory units and board & care facilities. Full time Monday – Friday 8am – 5pm.

The Nurse Practitioner Sr. is responsible for managing patient care and treatment (in person, telephonically and/or video) in collaboration with the physician. As a senior clinician, acts as a floater and provides coverage for nurse practitioners on PTO/LOA and also assists with high risk coverage. May also assist with new market launches. Primary duties may include, but are not limited to:

  • Performs health assessments and preventative health measures within prescribed guidelines and physician instructions.
  • Orders, interprets and evaluates diagnostic tests to identify and assess patient’s clinical problems and health care needs.
  • Discusses case with physician. Formulates and documents care plan.
  • Prescribes medication or other forms of treatment.
  • Participates in On-Call program, which may include after-hours, weekend and holiday calls and visitations.
  • Provides clinical coverage for nurse practitioners on PTO/LOA and may also support new market launches.
  • Assess nurse practitioner performance related to facility, to ensure appropriate relationships have been established with key staff, facility understands model of care, and knows who to contact for escalation if member has change of condition.
  • Mentors and coaches nurse practitioners and provides feedback on documentation and clinical decision making.
  • Is involved in onboarding and training of new nurse practitioners.

Qualifications

Requirements:

  • an MS in Nursing and minimum of 3 years of related experience with clinical emphasis for nurse practitioner; or any combination of education and experience, which would provide an equivalent background.
  • Current unrestricted RN license and NP license in applicable state required.
  • For CareMore business units, satisfactory completion of a Tuberculosis test is a requirement for this position. Travel may be required.
