Location: This position will cover the Hartford, Connecticut area.

About the Clinical Field Ops Team Lead role:  

  • Own responsibility for attaining clinical performance goals including hospitalization reduction, goals of care discussion, and home visit goals.  

  • Provide day-to-day oversight and management of all clinical staff in the market. 

  • Lead professional development and education efforts for a growing team of palliative care clinicians (NP, PA, SW, RN) including supervising training, performing ride-along visits with staff, mentoring/coaching staff, and implementing performance improvement plans for team members as needed.  

  • Interdisciplinary team (IDT) meeting focused on creating a positive team culture, discussing high-risk patients, providing clinical education and discussing operational needs.  

  • Establish and maintain positive, caring relationships with executives, managers, physicians, non-physician providers, support staff, and patients/families 

  • Assist with clinician recruitment, onboarding, and training.  

  • Assist with education and outreach efforts to primary care physicians and specialists.  

  • Assist with health plan communication and coordination.  

  • Assist with development and implementation of new policies and processes.  

  • Make as-needed urgent visits to acutely ill patients (please refer to general direct patient care responsibilities). 

 

Qualifications

Requirements:  

  • Family or Adult Nurse Practitioner OR Physician Assistant with appropriate certification and licensure in the state where you reside is required  

  • Possession of DEA registration or eligibility required  

  • CPR certification required 

 

Preferred Qualifications 

  • 2+ years of experience in home-based services, complex cases, and hospice and/or palliative care is preferred; some bedside hospital, oncology, ER or ICU experience is highly preferred. 

  • Experience working with EMR (Electronic Medical Records) systems is preferred. 

  • Excellent oral and written communication skills preferred to ensure that each of your interactions with patients, family caregivers, clinicians, and team members is effective and meaningful.  

  • Bi-lingual English / Spanish a plus.
